Effective use of "time before treatment" for critically ill patients

At present, in the field of cerebrovascular disease treatment, people are very concerned about "the time before treatment". That is, the most important period of time for the diagnosis and treatment of cerebral infarction due to vascular occlusion is the time before treatment.

If the disease occurs within 3 hours, the use of rt-PA thrombolytic agent can effectively improve the cure rate. If it is within 8 hours, Mercury Retriever, a thrombectomy device approved in 2010, can be used to remove blood clots that cause cerebral infarction. This thrombectomy device has been used clinically in Europe and the United States since 2003. In recent years, clinical studies have reported its effectiveness. The National Association of Cerebrovascular Disorders carries out a call for activities under the slogan "TIME LOST IS BRAIN LOST". The slogan means that the more time wasted before treatment, the longer the brain function will be lost the longer it is delayed.

However, subject to the medical system in Japan, few hospitals in Japan have a cerebrovascular specialist on duty 24 hours a day. Moreover, from ambulance transportation to hospital treatment sometimes takes a lot of time. In short, there are still many problems in the emergency medical system.

Mobile devices allow people to access information anytime, anywhere

In the treatment of cerebrovascular diseases, the patient's receiving system and correct early diagnosis are crucial. Therefore, our university carried out a project to develop a new remote diagnosis and treatment assistance system. The project was launched in October 2009, based on the concept of "can obtain patient information in any place inside or outside the hospital, and can seek expert diagnosis advice through the system", and aims to build a brand new medical information system.

First of all, the use of mobile information terminals that have developed rapidly in recent years, we take "no matter who, anytime, and anywhere" can receive information as the first goal, the remote diagnosis and treatment assistance system "i-Stroke" as a mobile Mobile phone) application software development (Figure 1). Strive to achieve the necessary images in emergency situations when necessary, through a high-security virtual private network (VPN: Virtual Private Network), so that each mode (CT, MRI, DSA, etc.) can be easily and quickly read (Figure 2). In addition, it is equipped with a system that can freely construct and view three-dimensional images-this is not available so far.

In order to realize these functions, we have worked with a Japanese company to develop an image preservation and communication system (PACS: Picture Archiving and CommunicaTIon System). The goal is to quickly forward and view images regardless of the circumstances.
